MAILBOX USE AGREEMENT

By filling out this form and signing below, I agree to the stated terms and conditions for use of a mailbox provided and maintained by LREPOA.

Terms & Conditions:

1) A mailbox will be assigned upon payment of a one-time $150 set-up and maintenance fee.

2) The assigned mailbox is for use of individuals residing at the address only.

3) Mailbox keys should not be copied (You will be given two keys).

4) Lost key(s) you will need to contact USPS for a replacement lock.

5) Mailboxes and/or keys are non-transferable; locks will be changed when lot ownership changes.

6) If mail is not removed from your box in 30 days, the Post Office may return your mail and we may reassign your box to someone else.

7) LREPOA is not responsible for lost, stolen or misplaced packages, mail, newspapers, etc.
